Here are all the details you need to feel confident and prepared.

Essential Forms for Your Camper

After initial registration, a few key forms are required for every camper before their first day of camp. These documents provide important information to help camp staff care for your child effectively.

Required forms include:

  • Future Stars Health Form: This provides essential health details about your child.
  • Immunization Records: A copy of your child's immunizations from their pediatrician is necessary.
  • Legal Disclaimer: This form outlines important terms and conditions for your understanding.
  • Medication/EpiPen Form: If your child has any prescribed medications or requires an EpiPen, a specific form must be provided. This form needs to be accompanied by a doctor's prescription, which should include dosage and timing instructions. You'll also need to bring the actual medication in its original bottle to camp.

Additional Forms for Specific Programs or Activities

For most campers, the forms listed above cover everything you need. However, certain programs or activities might require an extra waiver:

  • Program-Specific Waivers: Some specialized programs, like Adventure Camp, involve activities that require additional waivers. These programs may have more extensive documentation specific to their unique experiences. If you have any questions, contact your camp’s office for more information.
  • Field Trip Forms: If your child's program takes a field trip, an additional permission form will be required for the outing. Field trips will be communicated ahead of time to ensure everyone gets to participate.

When to Submit Your Forms

The sooner you submit your child's forms, the better! This allows camp staff ample time to review everything and prepare for your child's arrival.

All required forms should be submitted prior to your child's first day of camp. 

How to Find and Submit Your Forms

Finding and submitting the necessary paperwork is made easy for you:

  • Online Access: All required forms are available on the Future Stars website. Look for them under the "Forms and Rates" page.
  • Email Reminders: After you sign up, you'll receive emails letting you know what forms are required and if anything is missing. Reminder emails are also sent to help you stay on track.
  • Submission Methods: You have several convenient ways to submit:
      • Online upload
      • Email
      • Mail

If you need help or experience any technical difficulties with online submissions, simply contact your camp office for assistance.

What Happens If Forms Are Missing?

It happens! If your child arrives on the first day and forms haven't been completed, camp staff will work with you. You may be asked to complete the forms right then and there, or you'll be advised that they need to be submitted as soon as possible. Without the proper forms or waivers, your child may not be allowed to participate in specific activities. 

Guidance for Returning Campers

If your child is attending multiple weeks or sessions within the same summer, you only need to submit their forms one time for the entire summer. The only exception would be if a new program or field trip requires an additional waiver.

However, new forms are required every year, even if your child is a returning camper from previous summers. This ensures all information, especially health and medical details, is current and accurate for each new camp season.

Managing Medications at Camp

If your child requires medication during the camp day, rest assured it will be handled with care. Prescriptions are kept in the camp health office, ensuring they are stored safely and administered as directed by your child's doctor. 

An EpiPen, if required, remains with your child's group at all times.

Confidentiality and Sensitive Information

Your child's privacy and the confidentiality of their medical information are taken very seriously.

  • Limited Access: The camp health director reviews the information. It is then shared only with the specific individuals who need to know for your child's safety, such as their group leader and counselors.
  • Discretion and Privacy: Discretion is always used, and your child's privacy is maintained. Sensitive information is kept in your camper's private records, not in a general public database.
  • Emotional Concerns: An optional form is available if you would like to share any emotional concerns or issues your child might have, such as anxiety or trauma. This information can be helpful for camp staff, and it provides an additional layer of support for your child's well-being.

Understanding the Refund Policy

While planning for camp, it's helpful to be aware of the refund policy. After April 1st, there are no refunds for camp tuition. However, you do have the flexibility to change your child's registered week if needed.
